It's been four years since my father was a resident here in the memory care facility. He lasted 4 days before he was transported to hospice and died days later.

He had severe early-onset Alzheimer's and we had made the very hard decision to house him here. We felt comfortable with Dayna and the staff and the emotional toll had become overwhelming for our family to continue caretaking at home. It was especially hard that we were not able to visit him since it was amid the 2020 pandemic.

Once he got there, he essentially had a mental breakdown. They sent him to the hospital multiple times. Thankfully, Dayna was communicative with us, but we felt extremely helpless. One of the times I called for information, one of the head nurses answered and told me that she had worked in this environment for 10 years and had never seen anything like this.

To say that was not comforting, was an understatement. I ended up calling the Alzheimer's Association and they said that this was very normal behavior for someone in late-stage dementia. They were very comforting, unlike the head nurse here. It was a shame that even the "experts" did not know how to handle my father's condition. It is excruciating having to leave your loved one in the care of others. I hope this head nurse continues with more compassion in the future.

Additionally, I felt like they were overly eager to get him back into their facility even though he was told he had days left to live. The sales lady was more concerned about the check rather than our loved one who was dying.

The reason for two stars and not one is for Dayna - thank you for doing your best with my dad and for coming to his funeral. Thank you for caring.

I hope that this review helps others with their tough decision.

Newly remodeled community that has a really good feel to it. Connie at the front desk is so inviting! After 10 years of working there she is a prime example that great people wake up everyday to bring care and joy to her residents there at Harbor Chase Jax. Don't believe me, walk in and see for yourself 😊

My family experience was terrible. They couldn't provide the service we paid for.
There is too much to put. Not providing basic service, bathroom, grooming, showers, changing clothing.
Not coming to the room for two hours after buzzer was pushed with my father on the floor or not explaining how a cut on the foot happens and not properly cleaning it or not giving medication at the correct time or at all. The biggest issue is that they are under staff.
Have everything in writing and time stamp.
I do not recommend HarborChase.
We did get a refund, and some staff were let go, but unacceptable.

HarborChase has been a wonderful experience for my mother. The staff is always friendly and attentive. I looked around at other ALF and HarborChase felt most like a home with it homey decor and no other place has so many fun and interesting activities. My mother is very active and loves it!

My sister has been at HarborChase of Jacksonville for about 2 months now. She started out in respite care and is now a resident. Although she is younger than the average resident there, she feels very comfortable and cared for as she goes through a period of recovery and learning how to be more independent again. I have needed to assist her from another state, and that was a concern of mine not being in Florida close to her geographically. But the staff at HarborChase has never let me down. They always respond to any question or need that I have about my sister's care quickly and thoroughly and they always treat her with kindness and respect, going above and beyond to make sure she is receiving what she needs. I appreciate the staff and their attention to detail very much, and trust that my sister is in a safe and caring environment.

My precious mom, Della Easterday spent her final months in the loving care at HarborChase of San Pablo, in Jacksonville, Florida. The staff and facility provided her comfort, and support. Rebecca, Janann and Dana along with many others did the same for my family. I am forever grateful for this amazing facility and I still frequently go and visit the staff and residents. My Mom fell needed rehab and time to recover, I visited several assisted living facilities, and am so thankful I visited HarborChase, the place is beautiful, has great food and a has very open, friendly and homey atmosphere, Nick was so helpful with helping me get my Mom transferred and settled in her room, she has staff helping with rehab, and always comments how so kind to her and attentive to her needs everyone there is... We are very happy with chosing HarborChase and recommend visiting HarborChase if you or your loved ones have been looking for the right place..
