Where do I begin? I was there for a year and a half. I recently came home so I have a lot of insight about this facility first off a lot of the good reviews that you see that have been posted within the last couple months. These are forced on families within their first week of being there, however when you are there for an extended period of time such as I was, you get to see the facility in a different light. I will say that the new administration that they brought in our trying their best to improve the facility, but they are still a lot of room for improvement. The food is not bad and the facility is clean for the most part . One of my issues was the Inconsistency in staff Which In the end, the residence pay the price. Sometimes some of the CNA’s have anywhere from 13 to 20 people to take care at one time. This is nearly impossible with all the a.m. bathing care getting dressed making sure the residents get up and out for meals as well as cater to the families. There was several times when I wrote grievances and nothing never happened. The only way your family will survive is if you stay on top of the staff. That includes the doctors nurses and the CNA’s. While I was there, there was a lot of cover-up from medication, not being given to the correct person Medication’s not given at all, there were a lot of falls that were covered up and not reported. The Director and the rehab department in my opinion is racist. In my opinion, a lot of the Administrative staff are racist. Which means if you were not white, you are not getting the best care possible. No one will go the extra mile for you, you would have to do a lot of yours stuff On your own Or rely on family members. The rehab department is all about the money and not the care of the residence. After they have gotten all they can get out of your insurance company they no longer want to deal with you. This is regardless of your needs the therapist want to do more. However, the Director, which is Sabrina will not allow them to Sabrina is nasty disrespectful and not kind to others, she is one of the staff that you would need to put your foot down with. The doctor that oversees and sees most of the patients I believe her name is Dr. Subariah Or something of that nature. She does not listen to your concerns, she seems to overt you when you’re trying to Your symptoms and she is not willing to listen to your concerns and needs. She is another doctor. Who you may need to curse out! She barely knows any of her patients or what’s going on with them. She’s too busy with the administrator staff. Just do yourself a favor, and try to find somewhere else more suitable for your family member

I would give a zero if I could! My mother was sent there on a Wednesday afternoon and the staff was okay with my mom sleeping and not eating or drinking anything. They said it is normal for the first few days. I asked them to run labs to see if her labs were normal because she wasn’t talking right. That was on a Friday. I went back on Saturday and my mother was almost none responsive at that point. I told them I wanted her to be sent to the hospital and when they checked her vitals her blood pressure was extremely high.
Got her to the hospital and they discovered dangerously high ammonia levels. She was having seizures and was unresponsive for 3 days. She would have died if I didn’t get her to the hospital when I did. A week later she is still in the hospital. Please don’t send your loved ones here.

I would give them 0 stars if possible .This place is not what they say they are.Lies..Very neglectful. Do NOT SEND YOUR LOVED ONES THERE.List my husband of 34 years due to their neglect .Office staff Very Unprofessional,nor empathic. Most very nasty and rude.Requested Medical file was refused .Had to Have a Lawyer. Request them .Polaris Milford.has something to hide.

I have read the reviews. Yes I was not 100% happy every day. However, the staff was wonderful at the end of my moms life. Ebony, the 11-7 nurse and I’m sad to say I don’t know the name of the CNA were fantastic! Kind to me making sure I was comfortable as well as my mother. Liz, the business manager was kind through the almost year my mom was there. Shawn from house keeping check on my mom every day! Edna as well! Jess the day CNA was absolutely amazing, and Carrie the activities director was very helpful, caring and supportive! Let’s face it, no one wants a parent in a nursing facility but! I am happy that my mom was at Polaris! So many people to thank!

Polaris Healthcare staff and employees has provided my father with excellent care when caregiving became too much at home . All of the staff has been caring and helpful . The nursing staff addresses all concerns in a timely manner . Family stays well informed . Excellent care !

Recently, I spent 10 days in Polaris. I had been in the hospital for 2 weeks and decided to transfer to Polaris for intense physical therapy rehabilitation. Everyone was professional and courteous. I highly recommend Polaris (the food is good as well).
